Newly Updated! Adolescent Immunizaton Tip Sheet: Now in English and Spanish
The Adolescent Health: Immunization Tip Sheet is a helpful handout for healthcare providers to use to communicate important immunization information to their adolescent patients during tele-health visits, yearly exams (sports physicals and school physicals), and routine office appointments. The patient education Tip Sheet is available on the NPHF website in both English and Spanish versions. It includes information for recommended immunizations for adolescents, common responses to immunizations, and what an adolescent should do if there is a response. It also includes helpful hints for making good healthcare choices, using six “S” words for adolescent health: Sleep, Stress, Safety, Sex, Support and Say No.

Starting Insulin - A Patient Guide
A series of 10 handouts for patients starting insulin
With the growing number of people affected by diabetes and moving from oral medication to insulin, the Nurse Practitioner Healthcare Foundation, in collaboration with the Association of Clinicians for the Underserved, has developed a series of 10 patient handouts for patients starting insulin. These are free for clinicians and patients.
The series is available in low literacy or easy-to-read (6th to 8th grade level) and very low literacy or mostly pictorial (3rd to 5th grade level), in both English and Spanish.
